Reasonable adjustments are the things that our workplace or business can put in place to get the best performance out of that person possible.
They are designed or should be designed not to take away a task from a person so they don't have to do their job, but they should be designed so that frictions or difficulties are either removed or worked around to enable the employee to do the best job they can possibly do.
